SELFSURVEYS.COM PRIVACY POLICY

FOR CLIENTS

SelfSurveys.com is a service of Universal Responses Corporation ("URC"), a Delaware Corporation. URC is committed to protecting personally identifiable information. The following sets forth URC’s privacy policy for Clients (as defined below) on the sites of URC located on the World Wide Web at SelfSurveys.com and SelfSurveys.net a (collectively, the “Sites”).

From Whom Does URC Collect Personally Identifiable Information?

URC has two types of users: (1) [“Clients”] who register to become a client to enable them to build and deploy surveys using the URC online survey tools, and (2) “Survey Participants” who respond to and answer surveys that have been built by Clients using the URC online survey tools, which surveys are deployed by Clients directly to Survey Participants (usually via an email containing a hyperlink).  This privacy policy pertains to the use of the Sites by Clients. If you would like to view URC’s privacy policy relating to general visitors of the Sites and/or Survey Participants’ use of the Sites, please visit our Privacy Policy for Survey Participants. In addition, if you are a Survey Participant, we urge you to visit the privacy policy of the individual Client for whom you are completing a survey.

What Information Does URC Collect?

Personally Identifiable Information

In the case of Clients, URC collects personally identifying information such as name, company name, address, job title, email, telephone number and facsimile number when a Client registers. Depending on the method of payment, URC may collect a Client’s credit card number for purposes of payment or refund via a protected internet connection.

Other Information

In addition to personally identifiable information, URC also automatically receives and records information on our server logs from Clients’ browsers including Clients’ Internet Protocol addresses, cookie information (further described below) and the page you requested (“Other Information”). URC also gathers some statistical data when you visit the Sites.  The exact information we collect may vary, but can include the date and time of access to our Sites, the Internet Protocol address of a computer requesting a page, the pages requested, the type of survey used and computer and connection information such as browser type and operating system.  We do not associate this statistical information with your personally identifiable information.

Information Obtained from Clients through Surveys Sent from URC to Clients.

Periodically, URC may develop and send surveys to Clients or potential clients. These surveys will be aimed primarily at improving the URC service. Clients are not obligated to complete such surveys

What Are Cookies?

A cookie is a small amount of data, which often includes an anonymous unique identifier, that is sent to your browser from a Web site's computers and stored on your computer's hard drive.  Each web site can send its own cookie to your browser if your browser's preferences allow it, but (to protect your privacy) your browser only permits a Web site to access the cookies it has already sent to you, not the cookies sent to you by other sites. URC uses cookies to help gather information to provide a better service. You can configure your browser to accept all cookies, reject all cookies, or notify you when a cookie is set. However, cookies must be enabled on your browser in order for URC to provide the best experience possible and the advanced functionality of the URC service.

What Does URC Do With the Information Collected?

Clients’ Personally Identifiable Information

URC primarily uses Clients’ personally identifiable information to contact Clients; however, URC may also use such information for other purposes. URC will not access the surveys developed by Clients and sent to Survey Participants and will not access the responses of the Survey Participants, unless requested by the Client.

Survey Participants’ Personally Identifiable Information

If you would like to read about what URC does with the information gathered from the Survey Participants responses to the surveys, please visit our Privacy Policy for Survey Participants.

Clients’ Other Information

URC may use the Other Information (described above) obtained from Clients’ Web browsers or from surveys sent from URC to Clients and potential clients in order to maintain and improve the URC service and to describe our user base to potential and existing partners, advisors and investors.

Information Obtained from Surveys Completed by Clients

If a Client chooses to complete the survey, then URC may use that Client’s personally identifiable information and other information in order to improve the Service, including without limitation further contacting that Client for follow-up questions, offers or suggestions.
